As hospitals become more electronic, areas where paper persists become more difficult to blend into new workflows. One of the hard-to-automate areas is patient-generated documentation, including informed consent form signatures, HIPAA acknowledgement form signatures and medical history forms.

Paper forms are used by even the most automated hospitals because they are easy and fast for patients to use, yet they create a lot of work behind the scenes to route to Medical Records, scan and index. Timeliness can also be a problem as some signatures need to be viewed prior to surgery but may not have been scanned into the hospital EHR yet.
